Type 5 acrocephalosyndactyly; marked by asymmetrical head due to craniosynostosis, polydactyly of digits with characteristic shortening of thumb and great toe, syndactyly of 2nd-3rd digits, wide-set eyes, and normal intelligence; autosomal dominant inheritance.
