Hyperadrenocorticalism caused by ectopic ACTH source (extra-adrenal or extrapituitary neoplasm) or adrenal hyperplasia. Manifestations include moon facies, buffalo hump, truncal obesity, peripheral muscle wasting and weakness, striae, hypertension, diabetes, osteoporosis, hypokalemic alkalosis, bruising, acne, hirsutism, emotional lability, and menstrual abnormalities. Dexamethasone test is used to determine source of ACTH; 2 g dexamethasone is given PO q6hr with 24-hour urine collection for free cortisol on 2nd day. Pituitary source of ACTH usu. shows suppression of free cortisol, and ectopic source usu. does not.
