Surgical approach to anterior pelvic column and anterior acetabular wall. Indwelling Foley catheter is placed to decompress the bladder. Curved skin incision is made medially from anterior iliac spine to 2 cm above the symphysis pubis; aponeurosis of the external oblique is incised from the anterior iliac spine to the midline and to 2 cm proximal to the external inguinal ring; conjoined tendon is cut away from outer inguinal ligament, and iliopectineal fascia is divided with preservation of the lacuna vasorum. The iliopsoas muscle is then bluntly dissected from the pelvic ring, and spermatic cord and external iliac vessels are dissected free and retracted using encircling Penrose drains.
