Used to describe open fractures. Type I, without major contamination, no major soft tissue stripping, wound size < 1 cm; type II, no major contamination, no major soft tissue stripping, wound size 1-7 cm; type IIIA, small wounds with segmental fractures or small wounds with major soft tissue stripping and comminuted fractures or a wound size > 7 cm or a wound resulting form high energy trauma; type IIIB, highly contaminated wounds or blast injuries or wounds requiring rotational or free flaps or major soft tissue injury with significant periosteal stripping, type IIIC, any open fracture requiring surgery to repair vascular injury.
