Primary fibrinolysis is a rare condition that occurs when the body releases an abnormally high amount of tissue plasminogen activator from tissues that causes diffuse and sometimes uncontrollable bleeding.
It can occur in sepsis, cardiopulmonary bypass, and transurethral prostate resection (TURP) which can cause release of urokinase from the prostate.
In bleeding suspected secondary to primary fibrinolysis, use of antifibrinolytic agents Amicar (aminocaproic acid) and Transamin (tranexamic acid) can be quite helpful.
