\nPathophysiology<\/strong>
\n1)<\/strong> due to infection with protozoa Plasmodium<\/em> (P. falciparum, P. vivax, P. ovale, P. malariae<\/em>) 2)<\/strong> vector is bite of Anopheles<\/em> mosquito 3)<\/strong> infection with P. falciparum <\/em>causes most severe disease 4)<\/strong> P. ovale and P. vivax <\/em>can stay dormant in body for years and cause disease relapses even after apparent cure.<\/p>\n

Signs and Symptoms<\/strong><\/p>\n

1)<\/strong> malaise 2)<\/strong> fatigue 3)<\/strong> headache 4)<\/strong> chest and abdominal pain 5)<\/strong> muscle aches 6)<\/strong> fever 7)<\/strong> nausea and vomiting 8)<\/strong> orthostatic hypotension 9)<\/strong> jaundice 10)<\/strong> palpable spleen 11)<\/strong> pulmonary edema 12)<\/strong> renal failure 13)<\/strong> mental status changes (including coma) 14)<\/strong> seizures (P. falciparum)<\/p>\n

Characteristic Test Findings<\/strong><\/p>\n

Laboratory<\/em> – 1)<\/strong> increased bilirubin 2)<\/strong> mild anemia and thrombocytopenia 3)<\/strong> hypoglycemia 4)<\/strong> lactic acidosis 5)<\/strong> diagnosis is by visualizing asexual parasite in peripheral blood smear under Romanowsky’s stain<\/p>\n

Histology\/Gross Pathology<\/strong><\/p>\n

1)<\/strong> intraerythrocyte organisms 2)<\/strong> rosetting of RBCs 3)<\/strong> “knobs” on RBC membrane owing to cytoadherence<\/p>\n

Biochemistry<\/strong><\/p>\n

1)<\/strong> female mosquito regurgitates during blood meal 2)<\/strong> organism travels to liver with asexual reproduction inside hepatocyte 3)<\/strong> hepatocyte ruptures and organisms reenter bloodstream and enter RBCs (clinical disease now evident) 4)<\/strong> organism consumes almost all hemoglobin, reproduces sexually, and ruptures the RBC (within 72 h) and more RBCs are infected, etc.<\/p>\n

Associated Conditions<\/strong><\/p>\n

heterozygosity for sickle cell, glucose-6-phosphate deficiency, and thalassemia confer some resistance<\/p>\n

Inheritance\/Epidemiology<\/strong><\/p>\n

1)<\/strong> one billion people infected worldwide 2)<\/strong> 2-3 million deaths yearly 3)<\/strong> in USA occurs in travelers to endemic regions and in immigrants<\/p>\n

Treatment<\/strong><\/p>\n

1)<\/strong> chemoprophylaxis for travel to endemic arease – mefloquine or atovaquone are first choices (1 week before until 4 weeks after exposure) 2)<\/strong> for mild cases and nonfalciparum etiology – chloroquine 3)<\/strong> for more severe cases and P. falciparum<\/em> cases – quinidine or mefloquine 4)<\/strong> eradication of mosquitoes and larvae<\/p>\n

1)<\/strong> classic malarial paroxysms are episodic fever spikes, chills, and rigors 2)<\/strong> there is no rash 3)<\/strong> there is no photophobia or neck stiffness
