Pathophysiology<\/strong><\/p>\n

inflammation of the pericardium of greater than 6 weeks duration<\/p>\n

Signs and Symptoms<\/strong><\/p>\n

1)<\/strong> retrosternal chest pain (classically described as improved with sitting up position and leaning forward and worse by lying flat) 2)<\/strong> chest pain referred to back and lateral trapezius 3)<\/strong> fever 4)<\/strong> friction rub – high pitched and scratching quality (best heard in sitting position during expiration) 5)<\/strong> pleuritic pain 6)<\/strong> pneumonitis 7)<\/strong> pericardial effusion and possible cardiac tamponade 8)<\/strong> positive Ewart’s sign<\/p>\n

Characteristic Test Findings<\/strong><\/p>\n

Echocardiogram<\/em> – 1)<\/strong> best test for diagnosis Radiology<\/em> – 2)<\/strong> enlarged cardiac silhouette “water bottle” EKG<\/em> – 3)<\/strong> ST segment elevation in leads V2-V6 4)<\/strong> inverted T waves after ST segments return to normal Laboratory<\/em> – 5)<\/strong> increase in CK<\/p>\n

Associated Conditions<\/strong><\/p>\n

1)<\/strong> infectious – HIV, coxsackie, hepatitis, pneumococcus, Strep, Staph, Legionella, TB 2)<\/strong> drugs – procainamide, hydralazine, phenytoin 3)<\/strong> trauma 4)<\/strong> autoimmune – lupus, sarcoid, scleroderma 5)<\/strong> Dressler’s syndrome 6)<\/strong> postradiation 7)<\/strong> uremia<\/p>\n

Treatment<\/strong><\/p>\n

1)<\/strong> bed rest 2)<\/strong> aspirin 3)<\/strong> NSAIDs 4)<\/strong> occasionally steroids 5)<\/strong> pericardiocentesis if tamponade threatens<\/p>\n

Tips for USMLE<\/strong><\/p>\n

1)<\/strong> Ewart’s sign is dullness beneath angle of left scapula 2)<\/strong> Dressler’s syndrome is pericarditis after MI 3)<\/strong> if chest pain and fever occur 10 days after a cold, think pericarditis Distinguish MI and pericarditis<\/em> – 4)<\/strong> both with ST segment elevations and T wave inversion – in pericarditis, T waves invert after ST is normal; in MI, T waves invert during ST elevation 5)<\/strong> no changes in QRS in pericarditis<\/p>\n","protected":false},"excerpt":{"rendered":"
