1)<\/strong>\u00c2\u00a0Wims tumor is malignant renal tumor in children
\n2)<\/strong> occurs as both a sporadic tumor and as part of three genetic syndromes<\/p>\n

Signs and Symptoms<\/h4>\n

1)<\/strong> most commonly presents as flank mass
\n2)<\/strong> abdominal pain
\n3)<\/strong> bowel and ureter obstruction
\n4)<\/strong> hematuria
\n5)<\/strong> hypertension
\n6)<\/strong> peritonitis from rupture and intraperitoneal hemorrhage of tumor<\/p>\n

Characteristic Test Findings<\/h4>\n

Radiology<\/em> – CT scan is diagnostic test of choice and shows a large renal mass<\/p>\n

1)<\/strong> pale tan cut surface with foci of necrosis surrounded by a thin rim of renal cortex

\n4)<\/strong> all syndromic Wilms’ tumors have nephrogenic rests<\/p>\n

Associated Conditions<\/h4>\n

1)<\/strong> WAGR syndrome – Wilms’ tumor, aniridia, genitourinary defects, mental retardation
\n2)<\/strong> Denys-Drash syndrome – Wilms’ tumor, glomerulopathy, and sexual phenotype disorders
\n3)<\/strong> Beckwith-Wiedemann syndrome<\/a> – Wilms’ tumor, gigantism, enlarged viscera, and macroglossia<\/p>\n

Biochemistry<\/h4>\n

1)<\/strong> WAGR syndrome – associated wtih deletion of tumor-suppression gene WT-1 on chromosome 11
\n2)<\/strong> sporadic Wilms’ tumor is more associated with deletion of gene WT-2 on chromosome 11<\/p>\n

Inheritance\/Epidemiology<\/h4>\n

1)<\/strong>\u00c2\u00a0Wilms tumor most commonly presents at age 1-3 years
\n2)<\/strong> earlier presentation has better prognosis
\n3)<\/strong> sporadic cases – 99% of cases; 10% are bilateral
\n4)<\/strong> familial cases – both autosomal dominant and recessive inheritance
\n5)<\/strong> occurs in 1\/10,000 births<\/p>\n

Treatment<\/h4>\n

1)<\/strong>\u00c2\u00a0Wilms tumor is treated with surgical resection with chemotherapy and radiation 2)<\/strong> cure rates are 75-90%<\/p>\n
