Hemophilia A is a disease of the blood<\/p>\n

Pathophysiology<\/strong><\/p>\n

congenital coagulation disorder due to a defect in factor 8 (synthesized in liver), which is complexed to von Willebrand factor (vWF)<\/p>\n

Signs and Symptoms<\/strong><\/p>\n

1)<\/strong> bleeding into soft tissues, muscles, joints, typically hours to days after trauma 2)<\/strong> mass effect of accumulated blood can cause compartment syndrome 3)<\/strong> most serious areas of bleeding are into oropharynx and CNS 4)<\/strong> often presents with diffuse bleeding at circumcision<\/p>\n

Characteristic Test Findings<\/strong><\/p>\n

Laboratory<\/em> – 1)<\/strong> factor 8 < 5% of normal 2)<\/strong> prolonged PTT (other bleeding tests are usually normal) 3)<\/strong> increased AST and ALT<\/p>\n

Histology\/Gross Pathology<\/strong><\/p>\n

1)<\/strong> hemarthrosis 2)<\/strong> synovial inflammation 3)<\/strong> erosion of articular cartilage 4)<\/strong> osteoarthritis 5)<\/strong> articular fibrosis 6)<\/strong> pseudophlebitis from venous congestion 7)<\/strong> ischemic damage to nerves<\/p>\n

Inheritance\/Epidemiology<\/strong><\/p>\n

1)<\/strong> affects 1\/10,000 males 2)<\/strong> transmitted in X-linked mode 3)<\/strong> female carriers occasionally have inactivation of normal X gene with major bleeding<\/p>\n

Treatment<\/strong><\/p>\n

1)<\/strong> concentrated factor 8 or cryoprecipitate (contains factor 8) 2)<\/strong> in mild cases can use DDAVP 3)<\/strong> aim is to get factor 8 levels to 25% to stop active bleeding or to 50% as prophylaxis for major surgery (often requires 2-3 weeks of therapy) 4)<\/strong> the presence of inhibitors to factor 8 should be checked before treatment<\/p>\n

Tips for USMLE<\/strong><\/p>\n

1)<\/strong> look for a patient who has an increased PTT and a normal PT, bleeding time, and platelet count 2)<\/strong> never use aspirin products in hemophilia A patients<\/p>\n","protected":false},"excerpt":{"rendered":"
