Beta thalassemia is a blood disorder<\/p>\n

Pathophysiology of Beta Thalassemia<\/h4>\n

1)<\/strong> abnormal conversion of fetal hemoglobin<\/a> to adult hemoglobin due to defective biosynthesis of globin chain
\n2)<\/strong> patients can be homozygous (major) or heterozygous (mine or trait) for the gene<\/p>\n

Signs and Symptoms<\/h4>\n

1)<\/strong> failure to thrive
\n2)<\/strong> jaundice
\n3)<\/strong> icterus
\n4)<\/strong> gallstones
\n5)<\/strong> pallor under tongue
\n6)<\/strong> shortness of breath
\n7)<\/strong> “chipmunk” facies
\n8)<\/strong> leg ulcers
\n9)<\/strong> congestive heart failure
\n10)<\/strong> inanition
\n11)<\/strong> recurrent infections
\n12)<\/strong> bone and joint abnormalities (especially ankle pain)
\n13)<\/strong> “hair on end” skull<\/p>\n

Laboratory<\/em> –<\/p>\n

1)<\/strong> increased unconjugated bilirubin
\n2)<\/strong> hemolytic anemia
\n3)<\/strong> hypochromia
\n4)<\/strong> microcytosis
\n5)<\/strong> increased levels of bF and HbA2<\/p>\n

Radiology<\/em> –<\/p>\n

6)<\/strong> osteopenia of foot
\n7)<\/strong> coarse trabeculations in ankle joints<\/p>\n

Histology\/Gross Pathology<\/h4>\n

\"beta<\/p>\n

1)<\/strong> hypochromic microcytic anemia
\n2)<\/strong> target cells
\n3)<\/strong> teardrop-shaped red blood cells
\n4)<\/strong> erythroid hyperplasia with erythropoietic deposits in liver and spleen
\n5)<\/strong> massive bone marrow expansion
\n6)<\/strong> hepatomegaly<\/p>\n

Associated Conditions<\/h4>\n

1)<\/strong> glucose intolerance
\n2)<\/strong> endocrine disorders
\n3)<\/strong> cardiac failure due to secondary hemochromatosis<\/p>\n

Biochemistry<\/h4>\n

caused by impaired globin synthesis my mRNA<\/p>\n

Inheritance\/Epidemiology<\/h4>\n

1)<\/strong> highest rate in people of Mediterranean descent
\n2)<\/strong> heterozygotes relatively common in India, central Africa, and Asia
\n3)<\/strong> especially common in areas where malaria is endemic
\n4)<\/strong> death usually by age 30 years if not treated with bone marrow transplanation<\/p>\n

Treatment<\/h4>\n

1)<\/strong> chronic transfusions
\n2)<\/strong> splenectomy
\n3)<\/strong> folic acid supplements
\n4)<\/strong> bone marrow transplantation<\/p>\n

Tips for USMLE<\/h4>\n

1)<\/strong> usually presents clinically around 6 months of age with a previously normal birthweight infant now in the lower 5%-10% in birthweight and height
\n2)<\/strong> beta thalassemia major is homozygous with significant clinical disease
\n3)<\/strong> beta thalassemia intermediate is homozygous with less severe clinical disease
\n4)<\/strong> beta thalassemia minor is heterozygous
