Surgical repair for neglected rupture of calcaneal tendon. Patient is placed in prone position with tourniquet on thigh. S-curve incision is made from mid-calf to lateral side of calcaneal tendon, sural nerve is retracted laterally, and inverted V-incision is made over central aponeurosis. Arms of incision are 1.5 X length of tendon defect (defect is measured with knee in 30 degrees of flexion and ankle in 20 degrees of plantar flexion). Flap is then pulled distally and sutured into place.
