Antibiotics should always be given to patients with acute variceal bleeding. Prophylactic dosing has been found to decrease the rate of infection, decrease the risk of rebleeding, and decrease mortality. Generally, a third-generation cephalosporin such as ceftriaxone (Rocephin) is used if there is no contraindication.
