System used to predict mortality in acute pancreatitis (following criteria vary slightly for gallstone vs. nongallstone etiology). At admission WBC > 16,000/ml; glucose > 200 mg/dl; AST (SGOT) > 250 U/dl; age > 55 years; and LDH > 360 U/dl. At 48 hours BUN increase > 5 mg/dl; serum calcium < 8 mg/dl; fluid deficit > 6 L; arterial Po2 < 60 mm Hg on room air; base deficit > 4 mEq/ml; and hematocrit decrease > 10%. Presence of 1Ð2 criteria indicates < 1% mortality; 3Ð4, 15%; 5Ð6, 40%; and > 7, 90%Ð95%.
