Neurologic deterioration presenting in childhood with slowly progressive distal muscle weakness. Followed by onset of autonomic deterioration with achalasia, profuse sweating, digit cyanosis, and orthostatic hypotension; due to defect in cholinergic innervation. Both autosomal dominant and recessive inheritance patterns exist. Treatment is supportive and involves surgical palliation of achalasia.
